Your duties in brief are :-
You will be responsible for the co-ordination, assessment, planning, implementation and evaluation of individual patient programs of care.
You will have a focus on risk assessment and risk assessment strategies, ensuring that the requirements of the care program approach are met.
Additionally you will support empowerment of the patients with particular regard to their dignity, rights and advice.
